In the state of Kentucky, the legal age for tattoos is 16. Parents or legal guardian are required to be present the day you get your tattooed or pierced. Simply print the proper release form, fill it out completely, and get it notarized with the state seal. You will need to bring the minors birth certificate,(one of the following as ID) state ID, drivers license, government issued passport (Guardianship papers, if living with grandma or someone other then parents) Just a signature isn’t quite good enough!
